29 CFR 1960.66(b): Where the employer failed to comply with the requirements under 29 CFR Part 1904, subparts C, D, E, and G, by failing to record a work injury or illness resulting in lost time as required by Subpart C, 1904.7(b)(3): (a) On or about November 13, 2020, for the employee (case no. 8115568) who experienced a work-related illness from SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2) while assigned to administer influenza vaccinations, conduct SARS-CoV-2 testing, and handle SARS-CoV-2 test specimens located at the HELP USA Genesis and the HELP USA Bonanza temporary shelters for homeless veterans, Las Vegas, Nevada. (b) On or about December 22, 2020, for the employee (case no. 8243375) who experienced a work-related illness from SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2) while performing work activities as a health aid screener at the east entrance located at 6900 North Pecos Road, Las Vegas, Nevada. (c) On or about December 29, 2020, for the employee (case no. 8346362) who experienced a work-related illness from SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2) while providing care to a SARS-CoV-2 positive patients in the expanded intensive care unit (ICU) located at 6900 North Pecos Road, Las Vegas, Nevada.
