29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about 8 April 2021, at the fuel dock, Cangrejos Yatch Club, Carolina, PR. Employees engaged in marinas related operations had been verifying visually the fuel level at the top of the 8,000 gal tank with no fall protection system in place such as guard railing and/or fall arrest system so as to protect them from injury.

29 CFR 1917.44(d): The employer did not direct motor vehicle operators to comply with posted speed limits and other traffic control signs or signals, and written traffic instructions: On or about 8 April 2021, at back and lateral roads, Cangrejos Yatch Club, Carolina, PR. Employees engaged in marinas operations and civilians were exposed to struck by and caught by or in-between hazards as evidenced by the lack of traffic safety controls/information such as missing speed signs and stop signs at appropriate locations and/or rumble strips/surface indentations and other critical areas to remind drivers of speed.
