In addition the designated competent person did not had knowledge of the make, model and manufacture of the vertical and horizontal lifeline cable system being use to ensure capability.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. a) On or about 04/08/2021, each employee working from approximately 100 ft. from the ground, on an electrical powered adjustable two point suspension scaffold system while utilizing inadequate fall protection were not trained on the hazards of falling, and procedures to follow to minimized the hazards.

29 CFR 1926.502(d)(5): Fall protection systems criteria and practices. Personal fall arrest systems. Snap-hooks shall be sized to be compatible with the member to which they are connected to prevent unintentional disengagement of the snap-hook by depression of the snap-hook keeper by the connected member, or shall be a locking type snap-hook designed and used to prevent disengagement of the snap-hook by the contact of the snap-hook keeper by the connected member. Effective January 1, 1998, only locking type snap-hooks shall be used. a) On or about 04/08/2021, the snap-hooks and the wire rope being used by the employees working from an electrical powered adjustable two point suspension scaffold system were not compatible and that the snap-hook was not a locking type snap-hook.
