Where CDZs are being used, the employer did not ensure that each employee had been provided training in the hazards associated within a controlled decking zone:  (a)	Pinnacle Constructors and Specialties, Inc. at 220 American Lane, Aspen, CO 81611:  On and before April 7, 2021 the exposing employer did not provide training to every employee that worked in a Controlled Decking Zone (CDZ) on the second level of a commercial building.  Three employees were installing decking at the leading edge near the exterior of the building when one of the employees fell backwards into an unguarded stairway opening and sustained serious injuries.  This condition exposed employees to fall hazards of approximately 14.5 feet and 24 feet.  (b)	Pinnacle Constructors and Specialties, Inc. at 220 American Lane, Aspen, CO 81611:  On and before April 7, 2021 the exposing employer did not provide training to every employee that worked in a Controlled Decking Zone (CDZ).  Employees installed decking on the second level of a residential building.  This condition exposed employees to fall hazards of approximately 14.5 feet and 24 feet.
