29 CFR  1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allowed, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) At the jobsite, on the garage roof, employees were exposed to a fall hazard of approximately 12-feet and greater when they used an aluminum extension ladder that was not set three feet above the roof.   b) At the jobsite, at the rear of the house, employees were exposed to a fall hazard of approximately 10-feet and greater when they used an aluminum extension ladder that was not set three feet above the roof landing.
