A construction coordinator was taking pictures and observing in-progress roofing work at heights up to 22 feet without the use of fall protection. The following condition exposed the worker to falls up to 22 feet.

29 CFR  1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.     a) Lifetime Quality Roofing & Construction, LLC located at 298 Baldwin Ct, Westerville, OH 43082:  On and preceding April 7, 2021, the employer did not ensure that the ladder used for access to the roof extended at least three feet beyond the surface being accessed.  A construction coordinator was utilizing a separated half of an extension ladder to access the lower section of roof of a two story residential home with the ladder side rails extended 10 inches beyond the eave and was not secured to the house.  The following condition exposed the worker to falls up to 9 feet.
