29 CFR 1926.416(a)(1):Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: On or about 04-09-2021 at the 86 Church Street, Wethersfield, CT job site: The employer permitted employees to work in such proximity to the 220 volt service entrance cable that they could come in contact with live electrical parts in the course of their work. The employee was working on an aluminum ladder that was laying on the roof and was located within 2 ft of the power line. The employer did not contact the power company to request a live wire protection (a free service) or effectively guard the live part.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about 04-09-2021 at 86 Church Street, Wethersfield, CT job sit: An employee who was painting on a 12 pitch roof of a residential home, was exposed to a fall hazard in excess of 15 ft, and he was not protected from falling by means of personal fall arrest system.
