The employees were exposed to a fall distance of approximately 8 1/2 feet from the roofs edge.  B) Antonio Lopez dba AM Roofing worksite located 2 West Crawford St., Toledo, Ohio, 43612:  On or about April 9, 2021, the employer did not ensure employees were protected from falls while installing shingles on the second story of a residential structure. The employees were exposed to a fall distance of approximately  20 feet from the roofs edge.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Antonio Lopez dba AM Roofing worksite located 2 West Crawford St., Toledo, Ohio, 43612:  On or about April 9, 2021, the employer did not ensure the side rails of an extension ladder being used on the side of the second story residential structure was extended at least three feet above the landing surface. Two employee were exposed to falls while using the ladder to access/egress the roof.
