29 CFR  1917.112(b)(1): Guardrails were not provided at locations where employees were exposed to floor or wall openings or waterside edges, including bridges or gangway-like structures leading to pilings or vessel mooring or berthing installations, which presented a hazard of falling more than 4 feet or into the water.  a) Owensboro Grain Company Marine Terminal  - 822 E. Second Street: On 04/09/21 and days prior, the end of the gangway that workers use to access barges was not protected with guardrails exposing workers to a fall of greater than four feet.

29 CFR  1918.35: Employees were working around open weather deck hatches which were not protected by coamings to a height of 24 inches (0.61 m), or by taut lines or barricades at a height of 36 to 42 inches (0.91 m to 1.07 m) above the deck.  a) Owensboro Grain Company Marine Terminal  - 822 E. Second Street: On 04/09/21 and days prior, workers were working on a barge that was being loaded without fall protection exposed to falling in open hatches.
