On or about April 13, 2021, at a work-site located at 6216 Sunset Ave, Independence, OH, 44131; during the installation of roofing material, the employer did not ensure employees working on the ground were protected from possible danger of head injury from impact, or from falling or flying objects, falling debris from working surfaces of heights approximately 9.6 feet.

29 CFR 1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. On or about April 13, 2021, employees were observed working on roof of a single family home at a job site located at 6216 Sunset Ave, Independence, OH, 44131. The employer did not ensure exposed employees were protected from falls by utilizing a PFAS or protected by any other form of fall protection system at an approximate fall height of 9.6 ft.
