29 CFR  1926.20(b)(1):The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  a) Dany Roofing Construction, LLC located at 16495 Square Drive, Marysville, OH 43040:  On and preceding April 14, 2021, employees were performing roof work on a single story commercial building at heights up to 21 feet without fall protection. The employer did not initiate and maintain safety and health programs related to the roofing work being conducted by employees.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   a) Dany Roofing Construction, LLC located at 16495 Square Drive, Marysville, OH 43040:  On and preceding April 14, 2021,  the employer did not ensure that employees performing roof work on a single story commercial building at heights up to 21 feet were utilizing fall protection. Roofers were installing shingles and other roofing materials at heights up to 21 feet without fall protection. The following condition exposed them to falls up to 21 feet.
