Facility: Employees performing sanding, drilling, and seaming glass panels used hazardous chemicals, such as CRL commercial strength glass cleaner (containing more than 10% ethanol, 5% 2-propanol, and propane) and CRL 920 Polishing Oil (containing 20-40% butane). The employer did not develop and implement a hazard communication program; on or about 4/14/2021. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (n) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: a) An employee working on the manufacturing floor seaming glass panels was exposed to continuous noise levels at 115% of the permissible daily noise exposure ( 8-hour time-weighted average sound level of 90 dBA) during the 499-minute sampling period on August 23, 2021, and was not provided with an effective hearing conservation program.; Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
