The employees were exposed to a fall of  13'3". Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  location: edge of roof on or about: 4/12/21  a) Employees were engaged in EFIS work at the edge of the roof. The employees, who were not provided with fall protection,  were exposed to a fall of between 17'and 20'. 29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:  location: front of building on or about: 4/12/21  a) Employees were working from a tubular welded frame scaffold. The working level of the scaffold was not fully decked. 29 CFR  1926.451(e)(9)(iv): Cross braces on tubular welded frame scaffolds were used as a means of access or egress by employees erecting or dismantling supported scaffolds:  location: front of building on or about: 4/12/21  a) Employees were erecting a  tubular welded frame scaffold. The employees were climbing the crossbraces for access. A built in ladder was observed on scaffolding in the front of the building but that section of scaffolding was not connected to the sections that the employees were erecting.
