29 CFR 1910.132(d)(1): The employer did not conduct job hazard analysis of the workplace to determine if hazards are present or are likely to be present: Fabrication area: Employer is exposing employees to hazardous conditions. The employees are permitted to work with pieces of equipment for which hazard assessments have not been conducted. On and about 4/08/2021 an employee operated a material handling piece of equipment which used a magnetic device for lifting steel members. The equipment did not undergo hazard assessment to address what safety factor to use in determining the maximum weight for which the magnets would be allowed to lift to prevent failure of the magnetic device during use. A steel member which had lost contact with the magnetic lifting device struck an employee who was operating the lifting device. The employee was injured and was hospitalized after the employee was struck by the steel member which weighed approximately 538 pounds.
