29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:    a) On April 14, 2021, employees were exposed to eye injury while operating pneumatic roofing guns without eye protection during roofing work.   Relianz Restorations Co., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1438510, citation number 1, item number 1 and was affirmed as a final order on January 16, 2020, with respect to a workplace located at 12802 S. White Willow Dr., Building 6, Plainfield, IL 60586.  No abatement certification or documentation is required for this item.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On April 14, 2021, employees engaged in roofing activity were not protected from fall hazards while working at an elevation of approximately 12 feet above the ground.  Relianz Restorations Co., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1438510, citation number 1, item number 2, and was affirmed as a final order on January 16, 2020, with respect to a workplace located at 12802 S. White Willow Dr., Building 6, Plainfield, IL 60586.  No abatement certification or documentation is required for this item.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:   a) On or about April 14, 2021, the employer did not maintain records or certification of fall protection training for employees exposed to fall hazards.  Relianz Restorations Co., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(b)(1), which was contained in OSHA inspection number 1438510, citation number 2, item number 1, and was affirmed as a final order on January 16, 2020, with respect to a workplace located at 12802 S. White Willow Dr., Building 6, Plainfield, IL 60586.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
