The employer did not initiate and maintain safety and health programs related to the roofing work being conducted by employees.

Roofers were installing shingles and other roofing materials at heights up to 19 feet without fall protection. The following condition exposed them to falls up to 19 feet.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. a) Arriaga Roofing , LLC located at 840 College Ave. Westerville, OH 43081: On and preceding April 15, 2021, the employer did not ensure that the two ladders used for access to the roof extended at least three feet beyond the surface they were accessing. Roofers were using two extension ladders to access the roof of a two story residential home with the both ladders extended 15 inches beyond the eave and was not secured to the house. The following condition exposed them to falls up to 19 feet.
