29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 m) or more above lower levels were not protected from falling by a guardrail system, safety net system, or personal fall arrest system: (a) Jobsite, 934 Watersonville Road, Cowansville, PA: On or about April 14, 2021 an employee was spraying foam insulation in the upper room about the garage when they stepped into an opening in the floor that measured approximately 4 ft. wide by 12 ft. long. The opening was not protected by guardrails and the employee was not utilizing a personal fall arrest system. The employee fell approximately 20 ft. to the basement area.
