29 CFR  1926.451(g)(4)(i):Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:  (a) Jobsite located at 1000 W. Girard Ave. Philadelphia, PA - 19123: The employer did not ensure that an adequate guard rail system (including a top rail and end rail) was installed on the scaffold, thus exposing the employees to a fall hazard of approximately 14 feet while using the pulley system to haul brick siding material to the upper level of the scaffold.   This condition was observed on or about April 15, 2021.
