29 CFR  1926.451(c)(2)(i): Employer did not ensure scaffold footings were level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement.  New home construction located on Lot #E403 at 145 Royston Lane, Fayetteville GA.  Back side of the structure:  On or about 4/13/2021, the employer exposed employees to a fall hazard of approximately 14-feet  in that the work platform (tubular scaffold) showed signs of sliding off center of the support boards. This sliding is causing the scaffolding to show signs of not being plumb/level.

29 CFR  1926.451(b)(1): The employer did not ensure each platform on all working levels of scaffolds were fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(I)-(iii):  New home construction located at 145 Roycroft Lane Lot #E403 in Fayetteville GA. Rear side of the structure: On or about 4/13/2021, employee engaged in masonry activities were exposed to fall hazards while working from a 2 level platform scaffold at heights of approximately 14 feet from the top of the plank to the lower (ground) level. J & L Masonry LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1470740 citation number 1, item number 1 and was affirmed as a final order on 5/15/2020, with respect to a workplace located at (Lot A303) 145 High Street Fayetteville GA 30214.

29 CFR  1926.451(e)(1): Employer failed to provide safe access/egress to scaffold when scaffold platforms are more than 2 feet (0.6 m) above or below a point of access. Use of portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be provided by the employer. Cross braces shall not be used as a means of access:  New home construction located on Lot #E403 at 145 Royston Lane, Fayetteville GA.  Back side of the structure:  On or about 4/13/2021, and at times prior, the employer failed to ensure that a safe means to access or descend from a tubular welded scaffold system was provided to and/or utilized by employees. A ladder, whether free standing or a part of the scaffolding component(s) was not available to/for employees working from scaffolding during the installation of CEMEX.  J & L Masonry LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1470740, citation number 1, item number 3 and was affirmed as a final order on (5/15/2020), with respect to a workplace located at (Lot# A303) 145 High Street Fayetteville, GA 30124.

29 CFR  1926.451(g)(4)(i):Guardrail systems  was not installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.  New home construction located at 145 Roycroft Lane Lot#E403, back side : on or about 4/13/2021 and at times prior, the employer exposed the employees to fall of approximately 14 feet to the lower level, in that a guardrail system was not provided along all open sides of the scaffold platform as employees engaged in masonry work. J & L Masonry LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.451(g)(4)(i), which was contained in OSHA inspection number 1470740, citation number 1,  item number 4  and was affirmed as a final order on 5/15/2020, with respect to a workplace located at (Lot #A303) 145 High Street, Fayetteville GA 30214.
