29 CFR  1910.101(b): The in-plant handling, storage, and utilization of all compressed gasses in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks was not in accordance with Compressed Gas Association Pamphlet P-1-1965:    a. At the facility located at 1125 Cranleigh Ave, in Deland, Florida: On or about April 14, 2021, the employer exposed employees to struck-by and fire hazards, in that the employer allowed six unprotected, propane filled cylinders, to be openly stored in close proximity of forklift traffic (heavy objects) that could strike the cylinders.

29 CFR  1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors from sources such as open flames; lightning; smoking; cutting and welding; hot surfaces; frictional heat; static, electrical, and mechanical sparks; spontaneous ignition, including heat-producing chemical reactions; and radiant heat:   a. At the facility located at 1125 Cranleigh Ave, in Deland, Florida; On or about, March 20, 2021, the employer exposed employees to burn hazards, in that the employer did not take adequate precautions to prevent the ignition of flammable vapors, as the employer allowed employees to make homemade personal fire/warmth buckets in the work area, which included the hazardous ignition of flammable vapors, such as, but not limited to, alcohol.
