29 CFR  1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   a) Body shop area, on or about 4/16/21: Body techs using corrosive liquids for brightening aluminum wheels did not wear splash goggles or a face shield when diluting concentrated liquid with water. Employees were exposed to splash from corrosive during mixing operations.  Abatement certification must be submitted for this item.

29 CFR  1910.134(c)(1): The respiratory protection program was not updated as necessary to reflect those changes in workplace conditions that affected respirator use:  a) Throughout the facility, on or about 4/16/21: The respirator program developed for body techs and painters was not reviewed annually to reflect changes in the program including, but not limited to:    - selection and purpose of respirators used in the facility including powered  air purifying respirator and air purifying respirators.   - specifics on medical evaluations including providers, procedures, questionnaires and the identity of program administrator.       - specifics of fit-testing including methods used and providers.   - methods for employees to provide feedback on the effectiveness of the program.   Abatement certification must be submitted for this item.
