29 CFR  1910.147(d)(2): An orderly shutdown of machine or equipment was not utilized to avoid any additional or increased hazards to employees as a result of equipment stoppage:  a) CHS Refinery Laurel Montana, Coker Unit: On or about April 16, 2021, and at times prior, employees were exposed to caught in hazards from the unexpected energization of a Power Team (Model E, Serial #54010)  hydraulic pump and HYTORC (Model #5mxt, Serial#MSEA1428-106) torque head while performing maintenance on a 24" 300 pound piping flange. The employer failed to shut down the machine to perform servicing in accordance with their established procedure.
