29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift:  On or about April 8, 2021, An employee was exposed to a fall hazard on a construction site in Normal, IL, and the employer did not have a competent person conduct an inspection, in order to identify existing and predictable hazards.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about April 8, 2021, Employees performing trenching activities were exposed to a fall hazard when using an extension ladder on a construction site in Normal, IL, and the employer did not ensure the extension ladder extended at least 3 feet above the upper landing surface.
