29 CFR 1926.20(b)(1):The employer did not initiate and maintain accident prevention programs as necessary to comply with this part: a) CSB Roofing, LLC located at 7457 Williamson Lane, Canal Winchester, OH 43110: On and preceding April 19, 2021, employees were installing shingles and other materials for a re-roof project on a two story residential home at heights up to 25 feet without fall protection or PPE. The employer did not initiate and maintain safety and health programs related to the roofing work being conducted. CSB Roofing, LLC was previously cited for a violation of this occupational safety and health standard its equivalent standard 29 CFR 1926.20(b)(1), which was contained in OSHA inspection number 1501134, citation number 1, item number 1 and was affirmed as a final order on March 31, 2021, with respect to a workplace located at 6785 Oakton Ln Columbus, OH 43211

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) CSB Roofing, LLC located at 7457 Williamson Lane, Canal Winchester, OH 43110: On and preceding April 19, 2021, the employer did not instruct employees in the recognition and avoidance of unsafe conditions and the procedures to be followed in order to minimize these hazards. Employees were installing shingles and other materials for a re-roof project on a two story residential home at heights up to 25 feet without fall protection or PPE. Roofers were installing shingles and other materials on a roof that varied from 11-25 feet. The employees were not utilizing proper fall protection which exposed them to falls up to 25 feet. CSB Roofing, LLC was previously cited for a violation of this occupational safety and health standard its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1501134, citation number 1, item number 3 and was affirmed as a final order on March 31, 2021, with respect to a workplace located at 6785 Oakton Ln Columbus, OH 43211.
