The employer did not initiate and maintain safety and health programs related to the roofing work being conducted.

Employees were working at ground level and on the lower section of the roof beneath other workers and materials without utilizing hard hats. The following condition exposed the employees to struck-by hazards to the head.

Roofers were installing shingles and other materials on a roof that varied from 11-24 feet. The employees were not utilizing fall protection which exposed them to falls up to 24 feet.
