29 CFR  1910.29(b)(13)(i): The guardrail system opening used around holes that serve as points of access (such as ladderways),  did not have a self-closing gate that slides or swings away from the hole, and was equipped with a top rail and mid-rail or equivalent intermediate member that met the requirements in paragraph (b) of this section:   The employer is failing to protect employees from fall hazards over 4 feet. This was most recently documented on April 14, 2021, at the worksite located at 299 Extension Street, Verona, Missouri. Self-closing gates were not installed around holes that serve as points of access (such as ladderways), did not have a self-closing gate that slides or swings away from the hole, and was equipped with a top rail and mid-rail or equivalent intermediate member that met the requirements in paragraph (b) of this section. Instances include but are not limited to:   a. Elevated working platform in V-3 dry choline dryer access.  b. Elevated working platform in V-3 dry choline cyclone access.

This was most recently documented on April 14, 2021, at the worksite located at 299 Extension Street, Verona, Missouri. A flexible electrical extension cord was observed running through a doorway from v-10 1/2, through another doorway at building v-10.  The employer is failing to protect employees from serious fire, electrocution, electrical shock and burn hazards related to improper use of a flexible cord. This was most recently documented on April 14, 2021, at the worksite located at 299 Extension Street, Verona, Missouri. A flexible electrical extension cord was observed running through a doorway from v-10 1/2, through another doorway at building v-10 exposing the cord to abrasion damage that will compromise the exterior and interior wiring insulation.
