29 CFR 1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) Neil's Excavation and General Contracting, Inc. at 121 E 1st St. Cortez, CO 81321: On or before 4/21/21, the creating and exposing employer did not ensure that the 4'10" deep excavation was equipped with a ladder or other approved form of egress. This condition exposed employees to excavation cave-in hazards. Abatement Note: Abatement Certification is required for this item. See enclosed "Certification of Corrective Actions Worksheet."
