29 CFR 1910.304(g)(5):The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: On or about April 6, 2021, the Mud Engineer's trailer #1092 was not grounded. The setup crew did not insert the electrode rod into the ground; the electrode rod was found in-between the hitch frame and behind the air conditioner. This condition exposed employees to electrical shock hazards. Oil Patch Group was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.304(G)(5), which was contained in OSHA inspection number 1224641. citation number 02, item number 001 and was affirmed as a final order on September 13, 2017, with respect to a workplace located at Lease Covington 46sec Block 57T1, Well 10H, Orla, TX 79770.
