29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) OBS Property Maintenance, LLC - 3800 Lauer Lane, Dover, PA - an employee removing old roofing materials from a residential roof, approximately eleven (11) feet above a lower level, was not utilizing any means of fall protection, on or about April 18, 2021. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.502(d): The employer used body belts as part of a personal fall arrest system: a) OBS Property Maintenance , LLC - 3800 Lauer Lane, Dover, PA site - The five (5) employees replacing the roof were exposed to falls of up to twenty five (25) feet and were using body belts as personal fall arrest devices, on or about April 15, 2021. b) OBS Property Maintenance, LLC - 3800 Lauer Lane, Dover, PA site - There was one of the body belts being used as a personal fall arrest device was visibly torn and was not removed from service, on or about April 15, 2021. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
