(a) On or about April 14, 2021, at the worksite located at 6161 South 6th Street, Milwaukee, WI, the employer did not ensure that measures, such as wheel chocks or sand shoes, were used to prevent the transport vehicles from moving during loading and unloading operations, thereby exposing workers to fall hazards, struck-by hazards and caught-in-between hazards.

29 CFR 1910.28(b)(1)(iii)(C): When the employer can demonstrate that the use of fall protection systems is not feasible on the working side of a platform used at a loading rack, loading dock, or teeming platform, and work is done without a fall protection system when the authorized employees are not trained in accordance with � 1910.30. (a) On or about April 14, 2021, at the worksite located at 6161 South 6th Street, Milwaukee, WI, the employer did not ensure fall protection training was conducted for personnel who perform work operations on a loading dock greater than 4 feet above the lower level, thereby exposing workers to fall hazards. ...In the alternative... 29 CFR 1910.30(a)(1): The employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart before any employee is exposed to a fall hazard. And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.
