29 CFR 1926.20(b)(1):The employer did not initiate and maintain accident prevention programs as necessary to comply with this part: a) Ordonez Framing located at 1292 Chatham Ridge Rd, Westerville, OH 43081: On and preceding April 22, 2021, employees were installing shingles and other materials for a re-roof project on a two story residential home at heights up to 23 feet without fall protection, using improperly configured ladders no personal protective equipment. The employer did not initiate and maintain safety and health programs related to the roofing work being conducted.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) Ordonez Framing located at 1292 Chatham Ridge Rd, Westerville, OH 43081: On and preceding April 22, 2021, employees were installing shingles and other materials for a re-roof project on a two story residential home working at different levels without the use of head protection. Employees were working at ground level and on the lower section of the roof beneath other workers and materials without utilizing hard hats. The following condition exposed the employees to struck-by hazards to the head.

Roofers were installing shingles and other materials on a roof that varied from 10-23 feet. The employees were not utilizing proper fall protection which exposed them to falls up to 23 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Ordonez Framing located at 1292 Chatham Ridge Rd, Westerville, OH 43081: On and preceding April 22, 2021, the employer did not ensure that the extension ladder used to access the roof extended at least 3 feet beyond the upper landing. Employees were utilizing an extension ladder to access the lower roof with the side rails extending 24 inches beyond the upper landing without the ladder secured. The following condition exposed employees to falls up to 10 feet.
