29 CFR 1904.33(a): At this establishment, the employer did not retain (save) the OSHA 300 Log (or equivalent), the privacy case list (if one exists), the annual 300A Summary (or equivalent), and the OSHA 301 Incident Report (or equivalent) for five (5) years following the end of the calendar year these records covered.  a) Deptford Center for Rehabilitation and Healthcare: The employer did not retain the OSHA Form 300 Injury/Illness logs, OSHA Form 300A/Annual Summary, or the OSHA FORM 301 Incident Report, or their equivalent for the 2018 calendar year for five years, on or about 4/22/2021.  b) Deptford Center for Rehabilitation and Healthcare:  The employer did not retain the OSHA Form 300 Injury/Illness logs , OSHA Form 300A/Annual Summary, or the OSHA FORM 301 Incident Report,  or their equivalent for the for the 2017 calendar year for five years, on or about 4/22/2021.  NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
