29 CFR 1926.405(a)(2)(ii)(I): Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points: a) 6th floor, east side: Employees used electric powered hand tools for plumbing work. The tools were connected to a series of extension cords, which were routed through a window opening from a generator on the ground floor. An extension cord was pressed under a bundle of metal framing studs and was not provide with damage protection. On or about 04/20/2021.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: a) 6th floor, east side: Employees used an electric powered angle grinder for plumbing work. The angle grinder's power cord was not provided strain relief to its plug and its outer insulation was damaged. On or about 04/20/2021.
