Location: 2051 E 13th Street, Brooklyn, NY  On or about: April 22, 2021  a) Employees worked from the 2nd level of the tubular welded frame scaffold and the scaffold platform was not fully planked.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.   Location: 2051 E 13th Street, Brooklyn, NY  On or about:  April 22, 2021.   (a) Employees working from a 3-tier high tubular welded frame scaffold which had no means of safe access.  Employees climbed the frames and cross braces to access the work platforms.

Location: 2051 E 13th Street, Brooklyn, NY  On or about: April 22, 2021  a) Employees worked from a 3-tier high tubular welded frame scaffold that was not fully planked, and was not equipped with guardrails and had no safe means of access. The scaffold was not erected under the supervision and direction of a competent person.

Location: 2051 E 13th Street, Brooklyn, NY  On or about: April 22, 2021  a) Employees worked from a 3-tier high tubular welded frame scaffold without using any form of fall protection and  exposed to falling approximately 12 feet to ground.  Employees were standing on platform equipped with one or two planks and the scaffold was not equipped with guardrails or midrails.
