29 CFR  1926.102(a)(1):29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  a) Second floor, chase between bathrooms B-215 and B-216, on or about 4-22-21:  One employee was working as a Spotter, while another employee was drilling through a concrete slab with a rotary hammer drill, and the Spotter was not wearing eye protection.

29 CFR  1926.1153(c)(1):Specified exposure control methods. For each employee engaged in a task identified on Table 1, the employer shall fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1, unless the employer assesses and limits the exposure of the employee to respirable crystalline silica in accordance with paragraph (d) of this section:  a) Second floor, chase between bathrooms B-215 and B-216, on or about 4-22-21:  Two employees were involved in a concrete drilling operation, using a tool that was not equipped with a commercially available shroud or cowling and a dust collection system to minimize dust emissions, in accordance with paragraph vii of Table 1.  ABATEMENT CERTIFICATION IS REQUIRED FOR THIS ITEM
