29 CFR  1926.502(d): The employer used body belts as part of a personal fall arrest system:  The use of a body belt in a positioning device system is acceptable and is regulated under paragraph (e) of this section.   A) Roof(s) on front sections of house, on or about April 23, 2021, and or times prior to, employees working on a roof used climbing belts as part of a personal fall arrest system, thereby exposing employees to internal injuries from falls.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  A) Front of house, on or about April 23, 2021, an employee was using a metal  extension ladder to access the second floor roof of a house. The ladder did not extend three feet above the eve of the roof, thereby exposing the employees to a 20-foot fall hazard.  B) Front of house, on or about April 23, 2021, an employee was using a metal  extension ladder to access the porch roof of a house. The ladder did not extend three feet above the eve of the roof, thereby exposing the employee to a 10-foot fall hazard.
