29 CFR  1926.453(b)(2)(v):A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a)	Ken Myers Construction Co. on or about April 23, 2021, at the site location 2960 S Crissey Rd Monclova, OH. : The employer did not ensure that a worker operating a Ford utility truck #340 aerial lift basket connected a lanyard to the basket while conducting fiber optics cable installation on the shoulder of the road near a power and utility pole, thereby exposing employees to a fall hazard of approximately 20 feet.
