29 CFR 1910.179(l)(3)(i): Unsafe condition(s) disclosed during the frequent or periodic inspection(s) required by 29 CFR 1910.179(j) were not corrected before operation of the crane(s) was resumed: (a) Facility, Machine Shop, 100 Precision Drive, Harmony, PA: On or about April 27, 2021 the #3 10 ton crane was identified as having deficiencies during an annual crane inspection conducted by an outside contractor in October, 2020. The report stated that the bridge girder was twisted and rolled approximately 1 3/8" out of vertical level near the mid-span and should not be used. The employer "down rated" the crane from 10 ton to 5 ton and continued using it without having it repaired.

29 CFR 1910.215(b)(9):The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: (a) Facility, Machine Shop, 100 Precision Drive, Harmony, PA: On or about April 27, 2021 the Standard Electric Tool Co. floor grinder did not have the tongue guard adjusted to 1/4". The opening between the guard and wheel measured approximately 1". (b) Facility, Machine Shop, 100 Precision Drive, Harmony, PA: On or about April 27, 2021 the "AB" floor grinder did not have any tongue guards in place.
