29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) G. Herchler Builders, Inc. on or about April 27, 2021, at the site location of 1227 Cleveland Rd W Huron, OH: The employer did not ensure the side rails of the aluminum extension ladder used on site to access the ceiling joist of a covered porch were extended at least three feet above the landing surface. One employee working from atop a residential structure roof was exposed to fall from height of 11 ft. while accessing work area from ladder.

29 CFR  1926.20(b)(1):The employer did not initiate and maintain such programs as may be necessary to comply with this part.  a) G. Herchler Builders, Inc. on or about April 27, 2021, at the site location of 1227 Cleveland Rd W Huron, OH. The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards, such as falls during job site operations, decreasing the probability of employee injury and/or illness.   G. Herchler Builders Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.20(b)(1), which was contained in OSHA inspection number #1457688 citation number 1, item number1a and was affirmed as a final order on July 24, 2020, with respect to a workplace located at  407 Sassafras Dr. Vermillion, OH.

Herchler Builders, Inc. on or about April 27, 2021, at the site location of 1227 Cleveland Rd W Huron, OH. The employer did not ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems or alternative measures while performing residential framing from elevated surfaces.  G. Herchler Builders Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number #1457688 citation number 1, item number 1b and was affirmed as a final order on July 24, 2020, with respect to a workplace located at 407 Sassafras Dr. Vermillion, OH.
