29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Instance a. On or about April 20, 2021, an employee suffered a severe finger amputation while operating the "National Shearing Machine" where adequate machine guarding was not in place to prevent the hazard. Instance b. On or about April 26, 2021, the Leblond (SN#11C-1396X-2769) lathe machine was not adequately guarded where the lead screw guarding was not installed to protect employees from hazards associated with rotating parts. Instance c. On or about April 26, 2021, the Leblond (SN#15C-492) lathe machine was not adequately guarded where the lead screw guarding was not installed to protect employees from hazards associated with rotating parts.
