29 CFR  1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following 29 CFR 1910.147(c)(4)(ii)(A)-(D):  (A) A specific statement of the intended use of the procedure; (B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; (C) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and, (D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.  60 slitter, Yoder tension stand: on or about April 5, 2021, the control of hazardous energy procedures "Lock Out/Tag Out Document#: P406-A"  and "Lock Out / Tag Out Procedures   Hydraulic Unit - 60" Slitter   Lockout Disconnect #5" were developed for preventive maintenance and repair work performed by maintenance personnel. These procedures did not contain the required specific information for use by non-maintenance personnel assigned to perform carpet inspections, adjustments, and changes (setup) which exposed their hands to a moving ram caught-in hazard between the upper and lower boards (clamp) when equipment was not de-energized in correct sequence to ensure safety. These procedures failed to clearly identify all of the specific steps to be followed by employees to isolate and disable the tension stand in order to safely de-energize the equipment and control the hazardous hydraulic energy. The procedure also lacked specific steps to relieve hazardous hydraulic energy prior to equipment access and specific verification steps were not prescribed to determine the effectiveness of the energy control measures.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedure) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  (1) prepare for shut down by recognizing energy hazards and its controls, (2) turn off and shut down the machines, (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (4) affix lockout or tagout devices to each energy isolating device, (5) render safe all potentially hazardous stored or residual energy, (6) verify that isolation and de-energization of the machine or equipment have been accomplished.  60 slitter, Yodel tension stand - on or about April 5, 2021, the employer did not ensure that personnel inspecting, adjusting, and changing carpet on the clamp's upper and lower boards had isolated the hazardous hydraulic energy prior to beginning work. Because the equipment was not locked out, employees had their hands exposed to caught-in hazards from the unexpected clamp closing.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  60 slitter, Yoder tension stand: on or about April 5, 2021, inspections of the control of hazardous energy (lockout) procedure and reviews with authorized employees were not conducted at least annually to identify and correct any deviations or procedural inadequacies. Personnel assigned to perform carpet inspections, adjustments, and changes (setup) had their hands exposed to a moving ram caught-in hazard between the upper and lower boards (clamp) when equipment was not de-energized in correct sequence to ensure safety.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  60 slitter, Yoder tension stand: on or about April 5, 2021, the control of hazardous energy lockout/tagout authorized employee training was not provided to personnel to understand the purpose and use of the energy control procedure, and the methods and means necessary for energy isolation and control during carpet inspections, adjustments, and changes (setup) on energized equipment. Employees had their hands exposed to the moving ram caught-in hazard between the upper and lower boards (clamp) likely to result in serious injury.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.303(g)(1): Space about electric equipment. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.  Shop, southwest, wall-mounted Square D disconnect switch: on or about April 28, at least 30 inches width of working space about the electric equipment was not ensured due to other machinery obstructing access.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
