29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  On or about April 28, 2021, and at times prior thereto, under the belt conveyor, employees were exposed to caught-in, and struck-by hazards while performing servicing work under unguarded belts and pulleys without having an energy control program that included energy control procedures, employee training and periodic inspections.

In the batch plant where employees were exposed to electrical hazards from several flexible cords that were used to provide power to multiple components of the batch plant without permanent hard wired fixtures.  b. Outside of the storage shop where employees were exposed to electrical and tripping hazards from flexible cords that were used to provide power to a permanently fixed pump without a permanent hard wired fixture.
