On or about 4/23/2021, a) Employees worked on top the sidewalk shed and crossed onto the 2-point suspended scaffold using a 9.25 inch wide plank. The platform used to cross was not at least 18 inches wide.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access. Site: 1200 East 53rd Street in Brooklyn, NY. On or about 4/23/2021, a) Employees on site accessed the 2-point suspended scaffold platform using an unsecured plank placed in the guardrail of the suspended scaffold. The employees climbed over the railing of the suspended scaffold from a single plank approximately 10 feet above the ground.

The walkway to the scaffold did not have guardrail systems as required to protect the employees from falling.

The plank was not secured and and did not have the structural integrity to support employees' crossing. There was no competent person on site to inspect for the structural integrity of the plank.
