Location: Barton & Associates, Inc. at 149 Emerald Street in Keene, New Hampshire: On or about April 26, 2021, the employer did not record two instances, among other instances, of employee work-related COVID-19 infections that resulted in lost work days of 10 days each on the OSHA 300 log or its equivalent.
