(a) CKS at 6145 Purdue Dr. : On or about 4/15/2021 and at times prior, employees were exposed to amputation hazards in that employees were servicing a Marley SPX Cooling Tower (MD 60163AC2LOBE MDI0069806-A1 2013) by replacing a V Belt (V 4RBX144) and did not adjust the pulley to remove tension prior to belt placement.
