29 CFR  1910.22(a)(1):  The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition:  On or about April 28, 2021: a.  Employees working in the packaging room were exposed to trip and fall hazards from flexible cords for the conveyor belt, the Fuji stacking robot, and the fans lying on the floor.  b.  Employees working in the THMR mixer loading station were exposed to trip and fall hazards from flexible cords for the florescent bulbs and the World Weigh BWS scale lying on the floor.

At the loading station for the THMR Kwikcutter 449 mixer, the flexible cord for the World Weigh BWS scale was frayed and taped, exposing employees to an electrical shock hazard of approximately 220 V.

On the CME pellet machine, flexible cords were used in place of fixed wiring to supply power to auger motors.  b. On the 575ST Karsh mixer by the hammer mill, a flexible cord was used in place of fixed wiring to supply power to the Avery Weigh-Tronix XR 4500 scale.  c. At the THMR Kwikcutter 449 mixer loading station, flexible cords were used in place of fixed wiring for the World Weigh BWS scale, and the florescent bulbs.  d. In the packaging room, flexible cords were used in place of fixed wiring for the Fuji stacking robot, the conveyor belt, and the standing fans.
