29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about 4/29/2021, employee(s) installed new shingles atop the main upper roof of a two-story residential house at 41 Gold Street in Edison NJ, without using personal fall arrest system, such as body harness connected to installed safety line(s) anchored to roof peak; thus exposing them to a fall hazard of 17 feet from the main roof and 8 feet from the lower roof to the ground below.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about 4/29/2021, a Werner aluminum extension ladder was leaned against the rear south face of the two-story house at 41 Gold Street in Edison NJ and was used for access to the main roof.  The side rails were not extended at least 3 feet above the roof surface, exposing employee(s) to the hazard of falling 17 feet to the ground below.
