29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about May 3, 2021, employees engaged in towing and recovery operations were exposed to struck-by and crush hazards due to the employer not providing a program consisting of energy control procedures.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy source: On May 3, 2021; the employer did not ensure that employee(s) isolated and/or controlled the energy source by removing the ignition key of a semi tractor-trailer prior working underneath the tractor-trailer. This condition exposed employees to crushed-by and/or struck-by hazards.
