29 CFR  1910.134(c)(1):A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  (a)Dakota Kitchen and Bath, Inc at 4101 North Hainje Avenue in Sioux Falls, SD 57104: On or about and at times prior to April 22, 2021, the employer did not ensure that employees were protected from chemical hazards and hazards related to use of respiratory protection in that the company had not established and implemented a written respiratory protection program that included the provisions of  29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures.  This condition exposed employees to chemical hazards and hazard related to use of respiratory protection. 29 CFR  1910.134(g)(1)(i)(A):Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function:  (a)Dakota Kitchen and Bath, Inc at 4101 North Hainje Avenue in Sioux Falls, SD 57104: On or about and at times prior to April 22, 2021, the employer did not ensure that employees were protected from chemical hazards that were required to wear respiratory protection were clean shaven, and did not have facial hair that came between the face and the face piece of the respirator. This condition exposed employees to chemical hazards.   Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR  1910.213(c)(1):Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:  (a)Dakota Kitchen and Bath, Inc at 4101 North Hainje Avenue in Sioux Falls, SD 57104: On or about and at times prior to April 22, 2021, the employer did not ensure that employees were protected from laceration hazards in that the blade guard of the SCMI SI 3200 Slider Saw in the Parts Cutting Area did not completely enclose the portion of the saw above the table. 29 CFR  1910.213(b)(7):Feeder attachment(s) did not have feed roll(s) or other moving parts so covered or guarded as to protect the operators from hazardous points:  (a)Dakota Kitchen and Bath, Inc at 4101 North Hainje Avenue in Sioux Falls, SD 57104: On or about and at times prior to April 22, 2021, the employer did not ensure that employees were protected from pinch point hazards in that the Samco Mini Max power feeder (Model: 80 S/N: 0294) on the Power Matic Shaper (Model: #27-SHAPER S/T S/N: 0211027444) had a damaged housing that exposed a chain and sprocket.
