29 CFR 1910.22(d)(1):The employer did not ensure that walking-working surfaces were inspected, regularly and as necessary, and maintained in a safe condition. Building Number 01024, Receiving/Delivery Parking Lot: On and before April 27, 2021, employee(s) were loading /offloading semi-trucks and other vehicles in the parking lot with a Hyster H60FTM forklift. The parking lot had multiple potholes and uneven surfaces. The uneven surfaces and potholes could cause a forklift(s) to tip over and/or the forklifts' load(s) to fall. The employer did not ensure the parking lot working surfaces were maintained in a safe condition. Employees were exposed to crush and struck by hazards from the falling load(s) and/or forklift(s). Employees were also exposed to slip, trip, and fall hazards while walking.
